This assessment will enable you, the caregiver, to assess your loved one's capacity in the following areas: Homemaking, Driving, Health Management, Communication, Technology, Financial Management, and Legal Issues.

Please note, the assessment is to assess the capabilities of one person (not a couple). If you want to assess a couple, you will need to do it twice; once for each person.

But it can be used to focus attention on important areas of need. It is our hope that it will aid conversation between family members. Because the onset of frailty rarely occurs suddenly, it might be helpful to take the assessment once every few months and track the progress of your loved one.

You will be asked to choose a statement from four choices. One choice is not likely to be an exact fit. Choose the one that most closely describes your loved one's current situation. Don't overthink your answer; find one that is a close fit in the present moment.

You will then be asked to refine your answer by choosing whether the chosen statement closely fits your loved one's present circumstance OR if that person's present situation is somewhat (or a bit) worse than stated OR somewhat (or a bit) better than stated.

You will receive an immediate email with your current answers. We will then follow-up with a written assessment.
